New Form- Anyone know why?

It is specific to that doc; different practices each have their own pile of Medico-legal forms. Sounds like the practice is worried about a patient suing over being treated for pain using Suboxone. Many meds are used ‘off label’, without FDA indication; some docs want every off label use covered by a disclosure form, so you won’t come back and sue them by claiming you didn’t know it wasn’t FDA indicated.

Yes– buprenorphine IS indicated to treat pain … Alone as Temgesic or in the Butrans pach. But the medication ‘suboxone’ was not tested as a pain med and doesn’t have that indication. Similar example would be gabapentin used to treat migraine headaches, something pretty common– but that doesn’t have FDA indication for that use.
